Are you a registered NDIS provider?

Yes. Nurture N Revive is a registered NDIS provider (registration 4053413291), working to the NDIS Practice Standards with worker screening and incident processes in place.

What services do you provide?

Supported Independent Living (SIL), community participation, daily living and life-skills support, and one-on-one support work, at home or in the community.

What areas do you cover?

We support participants across Melbourne’s East and South East, based in Cranbourne, VIC.

Do you have a current SIL vacancy?

Yes. We have a place open in our 3-bedroom Supported Independent Living home in Cranbourne North. Contact us to arrange a visit and see if it is the right fit.

Can I visit the home before deciding?

Absolutely. We encourage participants, families, and coordinators to visit, meet the team, see the space, and ask the hard questions before committing to anything.

How do I get started?

Book a free consultation through our contact page, or call us. We will talk through your goals and the supports that fit, with no obligation.

Who can make a referral?

Support coordinators, participants, and families can all get in touch. Coordinators can use our dedicated referral form to send participant details securely.

How quickly will you respond?

A real person replies within one business day, and will tell you honestly whether a support or the vacancy is a likely match.

What funding and plan types do you work with?

We work with SIL delivered under Core supports, plus daily living, community access, and support work. We are a SIL provider, not the SDA housing provider, so SDA rent is arranged separately and we will explain how the two sit together.
