Nurture N Revive (ABN 70 663 765 580) is an NDIS registered disability-support provider, NDIS registration 4053413291, based in Cranbourne, VIC 3977. We are the entity responsible for the personal information collected through this website.
2. What we collect
We only collect information you choose to give us through our forms, plus limited technical data needed to run the site securely.
- Referral form:participant name or initials and email (required); and optionally the referring coordinator's name, organisation, phone, and email, the participant's suburb, postcode, NDIS number, and a short description of support needs.
- Enquiry form: your name, email, and message (required); and optionally your phone number and enquiry topic.
- Technical data: your IP address, recorded with each submission to prevent spam and abuse (rate limiting).
Please do not send sensitive case notes or detailed health information through these forms. We collect the minimum needed to respond, and gather anything further securely once we are in contact.
3. Why we collect it
We use the information you provide to:
- respond to your enquiry or referral;
- assess suitability for our Supported Independent Living vacancy and other supports;
- contact you about your request; and
- protect the website from spam and misuse.
We do not use your information for marketing, and we do not sell it.
4. Who we share it with
We do not disclose your personal information except to the service providers that help us operate this website:
- FormSubmit (formsubmit.co): a third-party email relay, based in the United States, that forwards form submissions to our inbox. By submitting a form, you acknowledge your information is transmitted through this overseas service for delivery.
- Hostinger: our web host, which stores form submissions in our database on its servers.
We may also disclose information where required or authorised by law.
5. How we store and protect it
Submissions are stored in a database on our hosting provider's servers, accessible only through a password-protected admin area. We apply reasonable measures — including access controls, abuse throttling, and bot filtering — to protect personal information from misuse, loss, and unauthorised access. No method of transmission or storage is completely secure, but we take these steps seriously.
6. How long we keep it
We keep personal information only as long as needed for the purpose it was collected — to respond to and act on your referral or enquiry, and to meet our records and legal obligations. When it is no longer required, we delete or de-identify it.
7. Accessing and correcting your information
You can ask to see the personal information we hold about you and ask us to correct it if it is wrong. Contact us using the details below and we will respond within a reasonable time. There is no charge to make a request.
8. Complaints
If you believe we have mishandled your personal information, contact us first and we will try to resolve it. If you are not satisfied, you can complain to the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
