NDIS plan reassessment: an evidence checklist for participants and coordinators

A plan reassessment is when the NDIS replaces your current plan with a new one. Good evidence, gathered early, helps the NDIS see how your supports are working. This checklist is for participants, families and support coordinators, and every step links to the NDIS page it comes from.

Checked against NDIS pages . Written by Nurture N Revive, a registered NDIS provider. This is general information, not advice on your plan. Your NDIS contact decides what they need.

The checklist

  1. 1.Expect a check-in 2 to 3 months before the reassessment date

Your NDIS contact normally reaches out about 2 to 3 months before your current plan’s reassessment date. They explain the kinds of information and evidence you will need. Use that window to request reports, because therapists and providers may need time to write them.

  2. 2.Think about the next three years, not only the last twelve months

The NDIS asks you to think about life changes you expect over the next 3 years, such as leaving school, starting or leaving work, or a change in your living situation. It also asks how connected you are to community supports, what your goals are and what plan length you would prefer.

  3. 3.Collect evidence that shows how your supports are helping

Evidence should show how your supports and services are helping you work towards your goals. Reports can also recommend supports you might need in future. If your living situation or support needs have changed, say so in writing.

  4. 4.Ask the people who support you for a report

The NDIS lists assessments or reports from treating healthcare professionals, therapists or support workers as examples of evidence. That can include your allied health team and the support workers or provider who see your daily routine.

  5. 5.Decide how you will send it

Reports can be submitted through the NDIS service hub or by mail. Support coordinators often help gather and send them. Confirm with your NDIS contact what they still need before the meeting.

  6. 6.Know what the meeting is for

A plan reassessment is the NDIS replacing your current plan with a new one. The NDIS page on the meeting explains what happens on the day and who can attend with you.
